The Bridge Support Community Rehab Pilot is a Third Sector provider with two rehab houses set in Kingston, and is a joint project with the South West London & St Georges MH NHS Trust.

The service aims to deliver specific recovery focused interventions to people with a severe and enduring mental illness, enabling individuals to maintain / develop their daily living skills and to understand and develop strategies to manage the symptoms they may be experiencing. This will include medication management and monitoring physical wellbeing. In this setting, the OT will act to support the staff of the residential units to ensure effective risk management, the development of personal goals and skills to maximise daily living, and support for move on to further independent living.

The team will provide community care to people with severe and enduring mental health problems who have been discharged from inpatient care. Clinical interventions will be intensive, but time limited with a focus on recovery and assessment for further placements. You will work closely with the Care Coordinator responsible for the clients. There is an expectation that clinical interventions will have an outcome. The assessments will be carried out using the Model of Human Occupation and treatments will be determined and carried out in the form of individual or group intervention.

Main duties of the job


  • To manage a caseload of clients with complex needs, using evidence based /client centred principles to assess, plan, implement and evaluate interventions.
  • To offer OT intervention for patients living at the SWLSTG Rehab Community Pilot Provision team caseload, being responsible for completing standardised assessments / manualised interventions (including groups) as agreed by senior professional lead
  • To provide leadership for junior staff, through supervision and appraisal.
  • To participate in the planning, development and evaluation of OT services within the Bridge Rehabilitation Team, holding responsibility for defined projects
  • To contribute to the maintenance and development of the Trust OT Service
  • To regularly supervise OT students on practice placement
  • To contribute to streamline working from admission to discharge in keeping with the principles in the Rehabilitation / Recovery Care pathway.
  • To promote recovery and social inclusion.
  • To act as the designated 'Disability Champion' leading the multi-disciplinary team in Disability Equality matters within the service and other areas as required.

Job responsibilities


  • To apply a high level of understanding of the effects of disability on occupational performance and provide training and advice on lifestyles changes and adaptations to the clients social and physical environment.
  • To promote social inclusion and physical and mental well-being using evidence-based practice. This is underpinned by the principles of hope and recovery, health promotion and relapse prevention.
  • To independently manage a clinical caseload of service user with complex mental health needs, demonstrating clinical professional judgement and liaise closely with care coordinators and team manager where relevant.
  • To identify occupational therapy goals as part of the overall care plan, using specialist mental health and occupational therapy assessments, including MOHO tools.
  • To consider both physical and mental health needs as part of the occupational therapy assessment.
  • To plan and implement specialist individual and/or group interventions collaboratively, to enable service users to achieve valued goals and satisfactory occupational performance outcome.
  • To identify any significant risk factors in line with local policies, with support from senior staff
  • To contribute to and work within clinical pathways and protocols.
  • To evaluate effectiveness of therapy interventions and quality of clinical decisions in conjunction with service users/carers, other professionals etc.
  • To promote and signpost individuals / carers towards the Recovery College and embed recovery into all interventions

Person Specification

Education, Training & Qualifications

Essential


  • Diploma or degree in Occupational Therapy
  • Registration to practice in the UK
  • Evidence of post registration training relevant to mental health practice
  • Training in the theory and application of the model of human occupation within the first 4 months of appointment

Desirable


  • Practice placement educators course within one year of appointment
  • Apple registration

Experience

Essential


  • Experience of carrying a caseload in a mental health setting
  • Experience of working in a multi-disciplinary team
  • Experience of maintaining a written record of continual professional development
  • Experience of individual and groupwork
  • Experience of writing specialist occupational therapy reports
  • Experience of participating in a service development project

Desirable


  • Experience of working in a range of mental health settings

  • Successful completion of preceptorship
  • Experience of being a practice placement educator
  • Experience of using mental health services as a service user, or carer, relative, friend of a service user.
