A new and exciting opportunity has arisen for a dynamic and innovative Occupational Therapist within the Cardiff Community Resource Team (CRT). The focus of the role is to independently lead, develop, plan, organise and evaluate the delivery of Occupational Therapy to those patients with complex needs, require management of a long-term condition and/or who require assessment and provision within the NHS Continuing Healthcare (CHC) criteria.


The post holder will assess, plan, implement and evaluate interventions for adults who live in residential nursing homes, private nursing homes, own homes and supported living accommodation


The post holder will contribute to the planning, development and evaluation of Continuing Healthcare and Occupational Therapy Services within the Cardiff Community Resource Team. This post is community based and involves travelling between a number of patient homes in the locality, as such, the post-holder will be required to be a driver with a valid UK driving license and daily access to a vehicle. The Cardiff Community Resource Team is based over two sites in Whitchurch and Llanrumney.


The ability to speak Welsh is desirable for this role, Welsh and/or English speakers are equally welcome to apply.
Autonomously managing a caseload of clients with complex needs, using evidence based / client centred principles to assess, plan, implement and evaluate interventions in primary care settings to include home visits and community facilities
Providing leadership for a clinical team to include responsibility for appraisal, supervision, induction, training and education.
Participating in the planning, development and evaluation of Occupational Therapy services within the Cardiff Community Resource Team, holding responsibility for defined projects.
Leading on the promotion and implementation of Local Government, UHB and RCOT strategies within the Cardiff CRT Occupational Therapy Team.
Contributing to the maintenance and development of the Occupational Therapy services within Cardiff CRT.
Demonstrating a commitment to the education of Occupational Therapy students ensuring the maintenance of clinical standards through regular supervision of students on practice placement.
Working in collaboration with the multi-disciplinary team and with primary care, voluntary organisations, local authority and other appropriate health and social care sectors to negotiate care provision across the primary/secondary care interface
Maintaining clinical records in accordance with legal requirements and professional standard.

Will hold a qualification in Occupational Therapy at degree level or equivalent.
Holds professional registration as awarded by the Health & Care Professionals Council (HCPC).
Evidence of recent formal training/ learning and continued professional development at a higher level, supplemented by short specialist courses.
Practice Educator Training Qualification.
Evidence of specialist post-graduate education

Post-graduate qualifications in a wide selection of clinical conditions.
Post graduate experience, including experience in the speciality or associated speciality relevant to post.
Inter-professional and multi-agency team working within both voluntary and statutory services
Knowledge and experience of working in community settings as an Occupational Therapist


Planning and prioritisation of workload.
Knowledge of current healthcare policies and application in clinical area in order to contribute to service development.
Provide timely intervention in accordance with service quality standards.
Presentations skills to include use of audio-visual aids.
Practice placement educator, responsibility for student education.

Knowledge and understanding of services that link across the primary/secondary care interface to provide integrated care. Management and application of Health and Safety at Work Act to include relevant Risk Assessments and application of the Manual Handling Policy.

Personal qualities
The ability to meet the demands of the role to comply with the Disability Discrimination Act.
Ability to travel to meet the demands of the post.
