As a Social Worker in the Mental Health Team, you will support adults with various mental health challenges, including anxiety, depression, and severe mental illnesses. Your responsibilities include conducting comprehensive assessments to evaluate client's needs, developing individualised care plans, and facilitating access to mental health services. You will emphasise person-centred support while ensuring compliance with statutory requirements under the Care Act 2014. Conduct thorough assessments of clients to identify mental health needs.
Make applications for Deprivation of Liberty Safeguards (DoLS) where appropriate, ensuring legal compliance and protecting individuals' rights
Work with a multidisciplinary team, including psychiatrists, psychologists, nurses, and other healthcare providers, to coordinate comprehensive care.
Maintain accurate, clear, and timely records of assessments, interventions, and care plans, ensuring all documentation is in line with council policies and legal standards
Relevant social work qualification
~2 experience in adult social work, including assessment and care planning.
