  • To be a core member of the Home First Planned Care management team

  • To provide operational and strategic, management and leadership to the Neurological Conditions Services which comprises of the Neuro Rehab Service (NRS), Neuro Clinical Specialist Team (NCST) and the Early Stroke Rehab Team (ESRT)

Main duties of the job


  • To ensure the excellent running of the service

  • To display good working knowledge of clinical standards and practices with particular regard to neurological conditions

  • To ensure the patient experience is central to service developments and activities

  • To apply working knowledge of neuro rehabilitation practices and ensure service department aims for the highest standards

  • To ensure patient pathways from referral to discharge are efficient according to demand and capacity challenges

  • To implement and evaluate audits to ensure the validity and reliability of processes

  • To ensure the services practice in accordance with CQC, aiming to achieve the highest standards

  • To deliver effective strategies to improve performance across a range of measures

  • Lead and participate in a range of staff and divisional meetings

  • To contribute a key leading role on interagency/multi-disciplinary team working To use specialist knowledge to inform service/policy developments as appropriate

  • To promote a high level of clinical effectiveness by use of evidence-based practice and outcome measures

  • To demonstrate advanced skills in dealing with complex issues to generate appropriate strategies for caseload and workload management of individuals andteams

  • To ensure arrangements for staff supervision, appraisal and mandatory training

  • Degree in relevant professional qualification
  • Current professional registration

  • Masters degree

Experience

  • Extensive post qualification experience in healthcare practice
  • Considerable recent experience in the field of neurological condition rehabilitation
  • Previous experience of complex caseload management
  • Previous experience of working in multi- disciplinary context
  • Previous experience of leading supervision, appraisal and personal professional development
  • Experience of planning and supporting service development activities
  • Experience in the design, implementation and review of audit projects
  • Experience of using patient feedback to inform service development

Desirable


  • Experience of resource management, and particularly using budgetary controls
