JOB SUMMARY :

As a Hospice Aide you will perform personal care activities related to the activities of daily living, under direction of a Case Manager [RN].

QUALIFICATIONS:

  • Must have reliable vehicle.
  • 12 months of caregiving experience required (home care, assisted living/memory care, skilled nursing/nursing home, hospital or hospice),
  • Hospice experience preferred
  • Excellent customer service skills
  • Current CPR Certified
  • Valid driver's license and auto insurance

RESPONSIBILITIES (including but not limited to):

  • Personal cares (bathing, dressing, grooming, toileting, skin care, etc)
  • Review patient care plan
  • Lifting and transferring
  • Assistance with range of motion & positioning, exercise & ambulation
  • Assistance with dining & nutrition
  • Electronic charting

In addition:

  • Needs strong understanding of professional boundaries
  • Must be able to lift 50 lbs
  • Good understanding of body mechanics
  • Strong communication skills-written, verbal, listening with attention
  • Good problem solving skills
  • Good time management of self and others
  • Able to function in an electronic medical record
