Bridgend Bridgend, United Kingdom Cwm Taf Morgannwg University Health Board Full time

An exciting and rewarding opportunity has arisen for one suitably qualified, enthusiastic and passionate Primary Care Nurse to join the Bridgend North Primary Care cluster.

We have 7 GP surgeries in the cluster, covering a population of approximately 47,000 patients and work collaboratively alongside community pharmacists, district nurses, the third sector and the local authority to support the health of our population.

The 'multi-disciplinary team' approach allows us to meet the health needs of our ageing population and is something we are already fully committed to and working with as a cluster. We believe this can bring huge benefits to our population with improved access to services closer to home and new and improved services and models of care for the population.

The Primary Care Nurse will work within the cluster multi-disciplinary team, providing support for individuals presenting with chronic conditions.

The Primary Care Nurse will also carry out Home Visits on behalf of the registered GP, to include nursing and residential care on behalf of the practices.

If you are dynamic, organised, work well within the multidisciplinary team and have an interest in developing a career in Primary Care, we would like to hear from you.

An induction programme and development/training opportunities will be offered.
The range of assigned tasks will be carried out with the highest standard to support and enable qualified staff to provide patient care, which reflects the quality of standards and mission of the HB. The post holder will be required to carry out a range of assigned tasks involving direct and indirect care for the benefit of the patient/client in accordance with agreed protocols. The range of responsibilities will involve rotation as appropriate to the needs of the service and will include the provision of clinical sessions across a number of chronic conditions including all the patients within care homes. The post holder will be responsible for ensuring that all policies and procedures are observed, and meet the HB requirements contained in the Health and Safety Policy. Will also be expected to assist in development of policies relevant to speciality.

The ability to speak Welsh is desirable for this post English and/or Welsh speakers are equally welcome to apply.

The role is permanent, however a secondment would be considered. Cwm Taf Morgannwg University Health Board is part of the NHS Wales family. Our Health Board provides primary, secondary and community health and wellbeing services to around 450,000 people living in three County Boroughs: We are a proud local employer around 80% of our 15000 workforce live within our region, making our staff not only our lifeblood of our organisation but of the diverse communities that we serve.
