Community Psychiatric Nurse

The service supports working-age adults who have moderate to severe affective and psychotic disorders. The teams offer support in a number of ways, drawing from the expertise of such a variety of disciplines.

You have a degree or relevant professional registration as a Community Psychiatric Nurse and will be an active member of a dynamic, highly motivated, skilled and creative team

You will be involved in the delivery of care-coordination to a specified caseload operating under the CPA framework, a daily duty service, ongoing risk management, care planning, support around benefits and housing, carers' assessments, physical health care and a wide variety of therapeutic interventions for which you can access training and supervision. You will also be involved in ensuring that the service is a positive learning environment for students from various disciplines. The role of a Community Psychiatric Nurse is to work in partnership with service users, developing their care plans and recovery plans to enable them to lead fulfilling lives. They will demonstrate an understanding of clinical interventions relevant to the locality teams protocols to include assessments, psycho-education, Risk management, relevant aspects of social inclusion, Recovery, care plans and discharge planning, working closely with carers, families and partner agencies to facilitate this. The post holder will work at all times to promote the safety and the well being of service users and their families/carers. The post holder will assess, plan and implement care, and provide specialist nursing advice and carry out specialist clinical skills procedures. They will provide clinical supervision to staff and students. The post holder will ensure that the locality NICE interventions and ethos are adhered to. Person Specification Education/Qualifications Essential
- Current NMC registration
- Evidence of continued professional development (CPD)
- Completion of Mentorship Course/ENB equivalent, or intention to complete.
- Significant post qualifying experience in working with those presenting with serious mental illness
- Experience of managing a caseload
- Previous experience of working in the community.
- Lived experience of mental health issues
- Ability to develop good therapeutic relationships.
- Excellent written and oral communication skills.
- Assessment, planning and intervention skills.
- Ability to travel across sites
- Demonstrate an understanding of the practices of Human Rights in the delivery of this role.
