We are currently recruiting into a full time Advanced Nurse Practitioner [ANP] post within the inpatient Unit at Oakwood Community Hospital, Berkshire. Oakwood Community Hospital is a 24 bed physical health ward within Prospect Park Hospital.


You will, work closely alongside medical staff supporting the delivery of care, which may include the community geriatrician, specialist doctor, GP's and other multi-disciplinary colleagues.


Whilst this post will be predominantly based at Oakwood Community Hospital, the post holder may be required to work at any of our other community hospitals in BHFT (West Berkshire Community Hospital and Wokingham Community Hospital) if needed, to cover shortages that may arise


Working as part of an integrated team, you will be critical in supporting the physical health inpatient units and play a lead role in supporting admission avoidance and the safe discharge of our patients into the community.


A part time contract may be considered for the right candidate
To practice at an advanced level of nursing, working as an autonomous practitioner within the multidisciplinary team including admitting and discharging patients to community inpatient units and onto the Rapid Response and treatment pathway in collaboration with the specialist doctor, Community Geriatrician and/ or GP.


~ Provide expert professional advice support and guidance to other members of the health care setting on the care required for patients.


~ To proactively manage patients with long term conditions and acute exacerbations of these, working collaboratively with the medical and nursing staff and be an advocate for shared care between professionals.

Post registration Advanced Clinical Study
Non-medical Prescriber.
Current NMC Registration.

Substantial post registration experience and two years' experience working at band 7 or equivalent.
Extensive knowledge of older people's and long-term conditions agenda.

Experience of working in primary or community health setting.
Experience of financial management.
Extensive knowledge of older people's and long-term conditions agenda.

Proven ability to identify, develop and implement risk.
Computer literate
Ability to travel to multiple sites as needed

Demonstrate an in-depth knowledge and experience of clinical examination, history taking, pharmacology, pathophysiology, etc.
Experience of multi-agency working across service interfaces.
