Hertfordshire Band 5 Triage Nurse

The CLCH Planned Care Team in the Herts Division is really proud of our new model for triage and initial remote assessment for patients being referred to the community nursing team. We are looking for nurses who are confident in talking to patients, carers, and family members on the telephone or via video consultation. You will be making a difference by providing initial clinical advice and information, signposting to appropriate services, and initiating episodes of community nursing care. Patients tell us that this first contact with the service is an important part of their whole package of care, because it gives them confidence that we understand their needs right from the start.

You will need to be confident using computers and creating electronic clinical records, as well as being familiar with the key areas of clinical practice being provided in the community.

This is not a remote working role and applicants are required to be office based with a team of triage clinicians at Hemel One, HP2 7YU.

Main duties of the job

This includes:

-assessment and care planning,

-management of complex wounds, leg ulcers, and pressure ulcers, drains, and negative pressure pumps,

-administration of medication including insulin, intravenous antibiotics, and symptom control at end of life,

-catheter management and bowel care

-identification of frailty, palliative care, and long term conditions management

-prescription of community equipment, continence products, and wound care supplies in line with local formularies.

The team operates 7 days a week, between 8am and 6pm. We have a variety of working patterns available, including 8am to 6pm, 10 to 6pm, 12 to 6pm. We have flexibility to employ full time and part time staff, with a minimum commitment of working 19 hours per week for substantive post holders. All members of the team are required to contribute towards a weekend rota, typically working three weekend days per 4 week roster period.

Successful applicants will be well supported through a comprehensive induction programme, a buddy scheme and receive mentorship from senior clinicians. You will have access to a variety of training opportunities delivered through CLCH Academy and be part of a wider support network who will support you in gaining a wide range of triage competencies. There will be a range of different opportunities on offer to develop your skills.

Job description

Job responsibilities


  • To give skilled nursing care, based on principles of best practice and clinical evidence. This includes: Chronic disease management, end of life care, continence promotion and wound management. Specific skills include phlebotomy, administration of medication via syringe drivers and intravenous therapy, PEG management, urinary catheterisation and tracheostomy suctioning.
  • Act as a Named Nurse with the support and supervision of the DN Team Lead. To take continued responsibility for the holistic health needs assessment including triage, signposting, appropriate implementation of health care and ensuring continuity of care following referral from acute care, rehabilitation units, GPs and other agencies.
  • Undertake comprehensive remote assessment of the physical, psychological and social care needs of patients using the Single Assessment Process and including the needs of carers. To provide individualised and person centred care plans that provide monitoring and promotion of self-care to prevent unnecessary hospital admissions
  • Act as the patients advocate and to facilitate the patients own choices with regard to nursing care, promoting independence and self-care, as appropriate.
