As a Social Worker in the Mental Health Team, you will provide crucial support to adults experiencing mental health challenges. Your responsibilities will include conducting in-depth assessments to identify individual needs, developing personalised care plans, and collaborating with various services to ensure seamless access to appropriate mental health support. You will prioritize person-centered, recovery-focused care while ensuring compliance with statutory duties under the Care Act 2014 and the Mental Health Act 1983. Conduct thorough assessments of clients to identify mental health needs.
Make applications for Deprivation of Liberty Safeguards (DoLS) where appropriate, ensuring legal compliance and protecting individuals' rights
Work with a multidisciplinary team, including psychiatrists, psychologists, nurses, and other healthcare providers, to coordinate comprehensive care.
Maintain accurate, clear, and timely records of assessments, interventions, and care plans, ensuring all documentation is in line with council policies and legal standards
Relevant social work qualification
~2 experience in adult social work, including assessment and care planning.
