Barnet Unplanned Integrated service works primarily as an admission avoidance service reducing the number of patients sent to hospital Emergency departments. We receive referrals from GP, LAS, 111, Residential and Nursing Homes to prevent admission of patients and support immediate discharge home from the emergency departments.

The Unplanned service also supports the early discharge home of patients on the D2A pathway 1 taking patients from the local acute hospitals.

We are a multi-disciplinary team consisting of Nurse Practitioners, Occupational Therapists, Physiotherapist, Rehabilitation Support Workers, and Technical Instructors with administrator support. This is an expanding admissions avoidance service working with both the primary care and the acute sectors to support and deliver treatment to patients in their own home. We are currently looking for a motivated and committed Band 6 nurses to join our team.

Main duties of the job

Candidates should be experienced nurse with sound clinical knowledge and proven experience in acute and community clinical environments. You will need to be adaptable to operate in a fast moving environment and have the clinical skills and ability to function as a lone worker in a larger team. Candidates should have excellent communication skills and the ability to provide care as determined by the clinical assessments. Candidates will have undertaken professional development and have extended nursing skills, with evidence to support this. CLCH offers excellent training opportunities and we can support staff to complete the Non-Medical Prescribing course and Advanced Nurse practitioner course. The role requires travel around the local community you will need to have a full drivers licence and access to a car. The service covers 8am - 10pm working 7 days per week. The shift patterns are 8am - 4pm and 2pm - 10pm fulltime including weekend working

Job responsibilities

  • To provide skilled, effective and evidence based nursing care to patients in a variety of community settings.
  • To assess referrals made to service, prioritise and respond according to priority, as appropriate both in terms of level and urgency. To respond within agreed timescales as appropriate.
  • To use enhanced skills and knowledge to assess physical and psycho social needs and develop a comprehensive and concordant plan of care with patients, carers and the wider Multidisciplinary Team as needed.
  • To make risk assessments, establishing the safety or otherwise of patients' ability to stay at home, or return home safely from another care setting, providing and arranging necessary nursing and supportive packages.

Qualifications

Essential

  • oRegistered Nurse with current NMC registration
  • oFull, valid UK driving licence.
  • oEvidence of recent and continuing post-registration professional education and training

Desirable

  • oExperience of working in Primary Care/ Accident & Emergency.
  • oExperience of assessing and providing quality care to people with Long Term Conditions
  • oQualifications in Community and/ or Accident and Emergency Nursing.
  • oQualification in teaching or practice-based learning.
  • oPhysical Clinical Assessment Skills Course or willingness to undertake the course.
  • oNurse prescribing qualification

Experience

Essential

  • Evidence of recent substantial post-registration nursing experience
  • Extensive range of clinical skills and experience including IV therapy,
  • Vene-puncture, cannulation, rehabilitation
  • Experience of working within a multi-disciplinary team
  • Experience of working with people with complex health needs
  • Experience of supervising junior staff and co-ordinating daily activities on a regular basis
  • Experience of undertaking comprehensive health needs assessment and writing care plans

Desirable

  • Experience of implementing evidence-based care in practice
