The successful candidate will be responsible for the delivery of a high level service in providing a proactive assessment, monitoring and management of patients over the age of 70 years with an SMI and the management of the team. As part of the national agenda for transformation the service will focus upon admission avoidance to both general hospital and a mental health inpatient setting by integrated working with the community teams within the local neighbourhood model in line with the North East Essex Alliance.
Responsible for the coordination and management of the team in providing a safe, effective and therapeutic clinical service, ensuring the delivery of high quality community mental health care packages.


Responsible for general service management including


Complaints/investigation management
Management of sickness absence/disciplinary/capability and other workforce procedures and policies.

Support others in their learning and development in relation to the management needs of patients with serious mental illness and frailty


To ensure the service works in line with CPA/Social carer/risk management/ safeguarding and other relevant policies.

The focus of this role is to lead, support and facilitate the development of integrated Neighbourhood SMI and SMI with Frailty pathways. You will have continuing responsibility for proactive assessment, monitoring and management of patients and ensuring care is current, evidence based and in line with practice standards. You will provide clinical leadership to the team in evaluation and monitoring of the service, provide specialist assessment as per the service users, and carers requirement
