Job Summary:
We are looking for an experienced practice nurse with a knowledge of chronic disease monitoring who can act independently in a variety of settings, making professional, autonomous decisions for which they are accountable. The role combines advanced nursing skills with leadership and strategic support to the wider leadership team. As a key member of the primary care team the Lead nurse functions in a variety of role dimensions to include, but not limited to, healthcare provision, clinical performance, research, team leadership and education. The Lead Nurse will also be actively involved in the planning and implementation of relevant areas of practice development.
SCOPE AND RANGE OF THE ROLE
Using specialist practitioner nursing skills to deliver the highest standards of nursing care in accordance with evidence-based practice and national practice protocols
To manage and develop a clinical acute and chronic case load, dealing with presenting patients needs in a primary care setting
To assist with the development of the nursing services offered by the practice
To provide clinical leadership within the nursing team
Review medication for therapeutic effectiveness, appropriate to patients needs and in accordance with evidence-based practice and national practice protocols
MAIN TASKS, DUTIES AND RESPONSIBILITIES
Clinical Role
Make professionally autonomous decisions for which he/she is accountable and provide safe, evidence based, cost effective, individualised patient care
Monitors, Interprets and evaluates patient test results in agreement with the patient and their GP, ensure appropriate course of treatment is initiated, or refer on if required
Delivers care and support planning and anticipatory care
Make appropriate referrals to primary, community and secondary health care teams
Delivers person centred care approaches including supporting self management and behaviour change, practising motivational interviewing, delivering compassionate care and adopting personal outcomes asset- and strength based approaches and co-production
Understands public health, including health inequalities, epidemiology and prevention
Utilises a range of skills elated to e-health and technology to enable care and support self management
Initiate appropriate investigations for patients suspected to have chronic disease i.e. diabetes, COPD, asthma, CHD, CVD, Heart Failure, Hypertension: referring to other clinicians as appropriate
Assist practice to develop and deliver specialist primary nurse led services including chronic disease management.
Assesses and manages long term conditions: has in-depth expertise in long-term condition assessment, including comprehensive clinical examination to address highly complex health needs and physical and mental health assessment.
Formulates an action plan for the treatment of the patient with long term conditions, synthesising clinical information based on the patients presentation, history, clinical assessment and findings from relevant investigations, using appropriate evidence-based practice
Manages programmes of care for patients with chronic diseases by planning, providing and evaluating care under agreed guidelines and patient group directions. Maintain disease registers in liaison with administrative staff. At annual review will assess patients understanding and ability to self-manage
Initiate and carry out programmes of health screening as agreed with the practice clinical team: promote health and wellbeing, giving general education and advice on weight, diet and healthy eating, activity and exercise, smoking cessation, alcohol consumption and chronic disease management
Implements non-pharmacological related interventions/therapies dependent on situation and technical requirements of care
Uses new knowledge in innovative ways and takes responsibility for developing and changing practice in complex and sometimes unpredictable environments
Undertake regular medication reviews for groups of patients deemed suitable by the clinical team
Provide up to date contraceptive and sexual health advice,
Administer child and adult immunisations and vaccinations in accordance with General Practice contract and following national and local programmes. Competent and up to date with anaphylaxis and resuscitation techniques.
Up to date and follow guidelines regarding cold chain ensuring safe storage, rotation, and disposal of vaccine and drug, and that adequate stock levels are maintained
Competent in procedures including venepuncture, BP management, urinalysis, blood glucose testing, peak flow technique, spirometry, pregnancy testing, cervical screening, wound management, taking swabs
Provide support and chaperoning to GPs when required including in minor surgery clinics and other procedures
Enables, supports and encourages individuals, families, carers and groups to address issues which affect their health and social well being
Contributes to the practice achieving GMS contract obligations
Maintain accurate records following NMC standards of record keeping
Ensure all data protection requirements are met when gathering, recording and storing patient data
Liaise and maintain good working relationships with all members of the practice team. Work within the MDT within the practice and across the wider health system

Provides a high standard of care at all times in accordance with the Code of Professional Conduct (NMC 2004).

Ensures patient confidentiality at all times in accordance with the Clinical Governance Framework and the Code of Professional Conduct (NMC 2004).
Adheres to the requirements of the General Data Protection Regulations (GDPR).
Communication:
Utilises highly developed communication, negotiation and interpersonal skills.
Communicates the patients health status using appropriate terminology, format and technology. The course of this communication process involves the discussion of complicated issues and the sharing of delicate information on a regular basis.
Provides health promotion and disease prevention services to patients who are healthy or have acute and /or chronic conditions by giving anticipatory guidance and counselling to promote health, reduce risk factors and prevent disease and disability.
Attends case conference in the course of duties.
Uses networks for the purpose of sharing relevant information in a safe and appropriate manner.
Education:
Participates in teaching of other members of staff within the surgery team is involved.
Participates in the mentoring process, coaching and supporting trainees in clinical development.
Lead Nurse
As the surgery's appointed Lead Nurse, assumes lead responsibility for:
Vaccinations and immunisations (including seasonal flu vaccinations) to include but not limited to:
Review and dissemination of vaccination and immunisation based education and updates to the clinical team;
Coordination of PSDs; and overseeing compliance with national requirements and service specifications, to include overseeing recalls supported by the nursing and administration team, in particular the Clinical Performance Manager.
Infection control to include, but not limited to:
Overseeing and updating the surgerys infection control policy, procedures and protocols;
Overseeing and arranging in house infection control training; and
Undertaking required infection control audits, producing associated reports and overseeing implementation of any required actions.
INR to include, but not limited to:
Overseeing and updating the surgerys INR policy, procedures and
protocols;
Proving in-house INR support and training; and
Overseeing appropriate operation and maintenance of INR Star.
Cold chain compliance to include, but not limited to:
Overseeing and updating the surgerys cold chain policy, procedures and protocols;
Undertaking required cold chain activity, audits, producing associated reports and overseeing implementation of any required actions.
Nurse Manager
Assume day-to-day responsibility for the operational (non-contractual) management of the surgery's nursing team to include, but not limited to:
Attending the surgery's daily huddle and reporting on staffing and clinics from a nursing perspective;
Preparing weekly nursing rotas to ensure good access to nursing clinics maintained;
Attending weekly leadership meetings and reporting on staffing, clinic and
other associated matters from a nursing perspective
Overseeing, managing and recording nursing team leave requests and absences, to include requesting cover in the event of nursing team absences; and
Subject to training and supported by the GPs and Practice Manager, undertake annual appraisals with the nursing team.
