Key Responsibilities:

  3. Provide skilled nursing care including wound care, medication administration, and symptom management.
  4. Deliver health education and support to patients and their families on managing their conditions, promoting health, and preventing complications.
  5. Coordinate care and liaise with other healthcare professionals, community services, and social care agencies to ensure continuity and integration of services.
  6. Advocate for patients' rights and preferences, respecting their autonomy and dignity at all times.
  7. Maintain accurate and up-to-date documentation of assessments, care plans, and interventions using electronic health records systems.
  8. Participate in ongoing professional development activities to enhance clinical skills, knowledge, and competence.
  9. Adhere to professional standards, regulatory requirements, and organizational policies and procedures.
  10. Contribute to service development, quality improvement initiatives, and research projects as appropriate.

Qualifications and Requirements:

  • Registered Nurse (RN) qualification with current registration with the Nursing and Midwifery Council (NMC).
  • Experience working as a registered nurse, preferably with experience in community or home-based care.
  • Excellent clinical assessment and communication skills.
  • Ability to work autonomously and collaboratively within a multidisciplinary team.
  • Compassionate, patient-centered approach to care delivery.
  • Knowledge of relevant legislation, policies, and best practices in community nursing.
  • Valid UK driving license and access to a vehicle (for roles requiring home visits).
  • Commitment to continuous professional development and lifelong learning.
