The Richmond Response and Rehabilitation Team (RRRT) are looking for two experienced band 6 community nurses or paramedics looking to broaden their experience within the UCR services in Richmond.

RRRT are keen to support any personal or continued professional development of nurses seeking to develop their skills and clinical expertise within the community.

RRRT aims to reduce avoidable hospital admission, reduce hospital length of stay and support patients at home in line with service objectives. This role works within a multi-disciplinary, multi-agency team and in partnership with primary health care and social care professionals, providing expert case management, clinical assessment, diagnosis and individualised treatment plans, promoting self-management.

The post holder will be responding to the initial crisis, acute exacerbation or initial diagnosis to support the patient at home. Close liason with MDT may include but is not exhaustive of: GPs, virtual wards, acute hospitals, district nurses, community matrons and social workers,

Main duties of the job

  • To be lead practitioners in undertaking Rapid Response cases within the Community
  • Carry out a range of nursing interventions, working within the scope of professional practice.
  • Provide expert management for patients, often with a range of complex long-term conditions, as part of the Richmond Response & Rehabilitation Service case coordination structure.
Working autonomously with advanced assessment decision-making and problem solving skills identifying and signposting patients to appropriate services

Job responsibilities

Assess and provide skilled nursing/ paramedic advice and/or care to all appropriate individuals receiving a service. Implement and evaluate programmes of nursing care

Use advanced skills and knowledge to assess, diagnose, plan care, initiate/ change treatment and discharge individuals and be able to determine if symptoms warrant further investigation and/or treatment

Demonstrate the use of advanced clinical knowledge to identify changing or deteriorating health conditions

Demonstrate awareness and application of clinical governance with particular attention to clinical effective practice, clinical audit, research, risk assessment, planning and management together with the prevention and identification of adverse events.

Person Specification

Experience

Essential

  • Post registration experience
  • Experience of undertaking complex assessment utilising advanced assessment skills
  • Experience of working with people with a long term illness
  • Experience of case management and providing complex multidisciplinary care which involves other health and social care professionals

Desirable

  • Experience of working in community setting

Qualifications and knowledge

Essential

  • NMC/ HCPC registration
  • Completion of Physical assessment course
  • Teaching qualification, Mentorship Preceptorship or equivalent
  • Knowledge of safeguarding Adults & Children

Desirable

  • Recognised qualification in long term conditions e.g CHD, COPD, Diabetes

Skills & Attributes

Essential

  • Understanding of current policies, guidelines, legislation and their implications for this role, including performance indicators and targets.
