Working in conjunction with the Clinical Nurse Manager, CCNT sisters, nursing staff, paediatricians, service users and the Directorate and Divisional senior management teams, the post holder will co-ordinate, prioritise, appropriately delegate and organise the safe and effective running of services for Children, Young people and their families working in the community delivering high level of nursing care to families with complex medical and nursing needs within the Huddersfield and Halifax area for the Childrens Community Nursing Team.

The post holder will provide a high level of clinical skill, experience and managerial leadership for nursing and support staff.

The department includes , Children's Community nurses, Children's Cardiology Specialist Nurses, Special School Nursing Team and other relevant MDT. The post holder will undertake delegated non-clinical duties to support quality, safety and service improvement in accordance with the Trust vision and values.

Main duties of the job

To maintain responsibility for the effective management of a caseload of children with complex health needs demonstrating assessment of self/time management in order to organise and prioritise cases and professional development.

To effectively co-ordinate the Childrens Community Nursing team service to ensure a seamless and timely pathway of care for children and families.

To provide care in the community to facilitate the prevention of admission to hospital and deterioration in health.

To work alongside the multidisciplinary team on a daily basis using resources efficiently to ensure the delivery of quality care to patients in the community setting and to supervise, induct and support the working of junior staff during the span of duty.

To provide nursing clinical expertise, leadership and education in community, competently supervising staff to manage complex cases. This includes supervising junior staff to gain competence in line with the Paediatric Nursing and Support workers essential training policy.

To confidently escalate in a timely manner to Clinical Manager, or Matron, (as appropriate) concerns about patient safety, quality and experience, offering solutions and evidence based recommendations.

Uphold the ethos of empowering children and their families to develop the knowledge skills and expertise to enable them to become independent and manage their own coping strategies wherever possible.

Job responsibilities


  • To take an active role in risk assessment within community nursing environment, supporting implementation of strategies to minimise risk. Ensuring incidents and near misses are reported through promoting a no blame culture.
  • To contribute to a culture that recognises and works in partnership with patients and their families, involving them in all decisions.
  • To provide assurance that individualised plans of care are formulated and implemented for all patients including assessment, planning, implementation and evaluation.
  • To ensure early intervention is provided to ensure the safe discharge of patients, working in collaboration with the medical team and ward areas with the aim of avoiding admission/ readmission.
  • To work within the NMC Code of Professional Practice, and within policies, procedures and guidelines of Calderdale & Huddersfield NHS Trust.
  • To ensure that high standards of nursing care in line with national and local policy and guidance are delivered and maintained.
  • To act accordingly when standards are not being maintained.
  • To recognise changes in a patients condition responding to and when the intervention of others is required and refer on as appropriate
  • To act as an advocate for the child, young person and their family, including promoting and protecting their interests, rights, equality, inclusion and privacy.
  • To be involved in and encourage participation in trust initiatives to improve patient safety and quality care.
  • To ensure exemplary infection control standards are maintained in care delivery.

3.2 Professional Duties

  • To be aware of and adhere to Trust Policies relating to the control and administration and safe storage of drugs.
  • To organise lead and participate in regular staff mandatory and essential skills updates and communication briefings
  • To be flexible to work within other areas or departments to meet the demands of the trust workforce model and changing service needs.
  • To provide mentorship and preceptorship to newly qualified staff, new starters and students, encouraging an effective and supportive learning environment.
  • To take responsibility for own continuing professional development and performance, including identifying own development needs and maintain own portfolio in accordance with revalidation requirements and mandatory and role specific requirements
  • To ensure exemplary standards of record keeping and provide assurance that this is maintained across all grades of staff in line with the recognised digital SOPs and locally developed guidance.

3.3 Managerial Duties

In accordance with the Standard Operational Policy-

  • To support junior and senior colleagues in their roles and be resilient to manage the changing needs of the service on a daily basis and in support of wider service requirements.
  • To manage concerns or complaints, inform ward managers and escalate when unable to resolve at ward level.
  • To participate in the investigation of incidents / complaints as required.
  • To establish and maintain effective communication, both verbal and written and relationships with multi-disciplinary team
  • To recognise, prevent where possible and manage areas of conflict, reporting to ward managers if unable to resolve at ward level.
  • To assist in the completion of junior staffs appraisals and implementation of actions ongoing.
  • Participate in research programmes utilising agreed research findings to enhance the patient and their familys experience
  • Actively participate in benchmarking clinical practice along with other areas.
  • Contribute to and participate in programmes of audit, including utilisation of results in practice.
  • Participate in and support the ward managers in improving Patient Experience, ensuring Childrens and young people and families are involved in decision making for their care.

Qualifications

Essential


  • Registered Children's Nurse
  • Specialist Practitioner at Degree level or evidence of study at degree level
  • Evidence of significant personal development
  • Comprehensive level of clinical competency

Desirable


  • Recognised teaching and assessing certificate
  • EPNS certificate

Experience

Essential


  • Extensive clinical experience within NHS at band 5 level or above
  • Experience of working in the community with children with complex health needs
  • Evidence of continual professional development
  • Undertaken or willing to undertake the Trust 'Work together to get results' programme
  • Proven experience in community paediatric care.
  • Evidence of further learning

Desirable


  • Evidence of ability to effectively co-ordinate an area
  • Able to demonstrate awareness of or evidence of clinical Governance
