We're seeking a dynamic Qualified Social Worker to join us in reshaping the future of elderly care in Monmouthshire.

Our charity arm pioneers safe discharges, respite care, and lifelong support for older homeowners.
Join a passionate team committed to empowering older adults with the care they deserve.
This is a new post, working both remotely and from the office in Pontypool, as well as visiting hospital settings in the surrounding areas.Coordinate safe and timely discharges, whether to home, interim respite, or residential care.
Manage your caseload with finesse, identifying needs and bridging service gaps.
Forge vital links with local teams and authorities, ensuring seamless transitions for We Expect:

Post-qualifying experience and Social Care Wales registration.
A degree in Social Work and up-to-date CPD.
It would be beneficial if you have worked within and have positive working relationships with the discharge teams and the local authority.Competitive salary: £38,000 basic + commission (OTE £44,000 pa) with a monthly car allowance.
