Support to be provided in the individuals own home, helping them get up and ready for the day, and supporting them within in community such as shopping, visits to the gym, and accompanying the individuals to other social activities.

*This is a part time role of 20 hours per week.*

*Your Role Responsibilities include:*
* Provide exceptional care and support to individuals
* Personal care which may include washing, bathing, showering and toileting

* Raise any safeguarding concerns
* Enable individuals to gain new skills and maintain independence
* Maintain daily logs electronically/paper of daily activities to a high standard
