The post holder will contribute to the implementation and delivery of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care (revised 2018) and be able to apply it within the context of assessment, review and identifying outcomes for the assessed needs of individuals, which will include assessing individuals with mental health needs and / or dementia.

The clinical assessor will be responsible for:
• Coordinating and undertaking Continuing Healthcare (CHC), Fast Track (FT) and Funded Nursing Care (FNC) assessments and CHC reviews for individuals across all aspects of care. this. This necessitates a high level of assessment skills requiring tact, diplomacy and the ability to liaise effectively with colleagues, health care and multi-disciplinary professionals, service users, carers and family members.
• Participate in duty desk and action requests appropriately.
• The timely review of all CHC, FT and FNC service users; functioning as a central point for referral, assessment, and support. This will inform the appropriate level of funding, the accuracy of data held by the CHC Service and the commissioning of appropriate CHC funded packages of care to meet the identified care needs of individuals deemed eligible for CHC.
• Participate in appeals and retrospective reviews, as part of planned competency development
• To line manage FNC clinical assessor and supervise students, Nurse associates etc.
