PURPOSE OF POST:
The following information aims to provide potential candidates with a better understanding of the requirements for this role.
The Adult Social Care Division exists to promote the independence, inclusion and well-being of all adults with social care needs. It aims to do this by enabling people to:
• Have control over their lives.
• Live safe, healthy and fulfilled lives.
• Have an active role in a stronger and prosperous community.
• Access the support they need in order to be as independent as they choose.
You will be expected to: 1) receive and co-ordinate referrals, undertake the resulting assessments which will predominantly focus on equipment/adaptations and may also include carers assessments; 2) coach and mentor Social Care Assessors and Rehabilitation Officers who are undertaking assessments, support planning and review tasks; 3) influence the direction of future services by promoting independence, enablement and rehabilitative models.
You will be expected to be able to cover for your line manager on occasion and to also cover for OT’s from other teams if required to do so.
PRINCIPAL RESPONSIBILITIES:
Complete professional strength based and holistic assessments of need and risk for service users and carers with highly complex needs using Luton Borough Council eligibility criteria. This involves:
- Undertaking statutory duties under the Care Act 2014 Mental Health Act and Mental Capacity Act other legislation and guidance relating to service users and carers.
- Overseeing a duty role as necessary and prioritising level of intervention (including allocating initial assessments to be undertaken by social care staff)as appropriate
- Undertaking moving and handling handling risk assessments. Advising on and providing equipment and techniques to reduce risks and promote customer autonomy and dignity, whilst ensuring a cost-effective service by implementing single-handed care where safe and appropriate.
- Working with other professionals and partners to achieve effective joint working through initiatives that require a multi-agency approach, including those involving member interests, Environmental Health or Housing (including producing information and data for reports and statistics), participating in and organising and chairing multi agency meetings
- Keeping up to date to with legislative changes and best practice in occupational therapy and providing advice to colleagues
- Keeping appropriate case records, statistics and information utilising Council computer case recording systems, and other IT systems where required
- To provide professional advice, support and education to staff across health and social care, and key stakeholders in the principles of single-handed care
Coach, develop, supervise and mentor a group of up to 4 unqualified staff (Social Care Assessors and Rehabilitation Workers). Providing effective leadership, support and challenge to enable them to undertake assessments, including risk assessments, complete Resource Allocation Panel paperwork and support service users and carers with support planning and review tasks. This involves:
- Ensuring that service users and carers are involved in timely assessment and support planning and review processes
- Ensuring needs are met through negotiation with service users and their carers and consultation with professional colleagues, and that Support Plans are person centred and focused on individual outcomes, meet assessed needs, address risks, promote independence and choice, respect service users dignity and rights and meet their cultural needs
- Checking that equipment that is recommended provides value for money
- Ensuring there is engagement with other professionals and partners to achieve effective joint working through initiatives that require a multi-agency approach
- Ensuring the provision of an effective service to local people by maintaining high standards of professional practice and implementing quality assurance systems
When necessary and subject to accreditation, carry out, when required the role of practice teacher for students on professional qualifying courses
Undertake research for specific items or problems and inform colleagues and service users/carers of the range of services available to meet needs and the likely benefit and cost of different options to maintaining independence. (This overlaps with tasks identified under 1 and 2 above and is intended to reflect the additional time this may require)
Contribute to the development of strategic plans and or borough wide and neighbourhood projects with professional knowledge on an operational and strategic basis, as required
Experience of working with adults with sensory Impairments
Experience of working with adults with dementia, sensory Impairments, mental health, autism, learning disabilities, physical disabilities
Experience of managing and supervising staff
