As our Support Coordinator, youll be at the heart of supporting our customers who have complex needs, and are at risk of homelessness, to develop their skills to live more independently in the community. Typical day as a Support Coordinator
Creating support plans with your customers and coordinating our small team of Support Workers working to the plans.
Helping customers to build their independent living skills by supporting them with budgeting, maintaining their tenancy and moving onto independent accommodation when they are able.
Carrying out risk assessments, support planning, goal setting and regular reviews.
Collaborating with colleagues to ensure the smooth running of the service and making service provision decisions in lieu of management where required.
Ensuring effective communication between management and the services support team.

You bring
Passion to support our customers to live their best life, working collaboratively with an eye for detail.
Experience of creating person-centred support plans and risk assessments
Experience ofsupporting colleagues in working to customer support plans, considering risk and mitigating this.
Experience of coordinating and assessing customer referrals.
And most importantly, the ability to workon your own initiative, remaincalm under pressure and havea resilient approach.

Job details
Flexible working hours where possible
Where were flexible with you, we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion.
Rota pattern of 8-hour shifts between 07:45am 20:15pm, five days per week
Flexibility to manage your own diary.
Although youll sometimes lone work, we do get together regularly for training and team meetings.
Able to use technology for creating and updating support plans, complete online learning and to collaborate with colleagues.
Youll need an Enhanced DBS check done and we pay for that.
