COMMUNITY NURSE 
Community Nurse ( Part time) 2/3 days per week.  2 days must be Thursday and Friday.  3 days Thursday and Friday plus one other weekday to be confirmed on job offer. 

Mainly office hours 08:30 - 16:30 though some flexibility to support staff and clients is required. 

We are always on the lookout for talented and passionate Registered Nurses to join our team, whether you are newly qualified, returning to practice or are looking for a change of direction. Experience of community nursing and/or complex care is preferred but not essential. Must have NMC PIN.

We care for clients in their homes, with complex needs, End of Life, and much, much more.

  • To act as a Named Nurse within the team, developing collaborative, needs based care plans, through assessment, planning, implementation and evaluation.
  • You will provide excellent professional, skilled and effective person centred, evidence based nursing care.
  • You will be actively involved in the health and wellbeing of staff and clients.
  • Visibly and accessible to patients, families and carers working together to ensure efficient, effective and caring services.
  • Contribute to achieving all performance indicators, essential standards and quality outcomes
  • You will contribute to the education and development of employees.  
  • Contribute to staff appraisals and support personal development plans.
  • Safely administer medication following company policies and practice guidelines.
