Main ResponsibilityAs a valued Health Visitor, you'll:
- Deliver the Healthy Child Programme, assessing health needs and implementing care packages.
- Use tools like the Family Health Needs Assessment to plan and evidence care.
- Create individualized care plans alongside families, tailoring them to each child's needs.
- Work closely with other professionals, taking on a lead role when appropriate.
- Support new staff and students, with guidance from our dedicated Practice Education team.

The Ideal CandidateYou'll need to be:
- A qualified SCPHN (Specialist Community Public Health Nurse) with current NMC registration.
- Experienced in leading interventions to improve outcomes for children and families.
- Passionate about driving positive change and open to sharing your ideas for innovation.
- A driver with a valid UK license and access to a car for work.
