You will play a vital role in empowering individuals to live as independently as possible, promoting dignity, choice, and inclusion. Key Responsibilities Provide person-centred support to adults with complex needs, including learning disabilities, autism, mental health conditions, and physical support requirements. Assist with daily living activities such as personal care, medication administration (after training), meal preparation, and household tasks. Support individuals to engage in community activities, education, employment, and hobbies. Promote independence by encouraging decision-making and developing life skills. Maintain accurate and timely records, including care plans, incident reports, and daily notes. Work collaboratively with families, health professionals, and external agencies to ensure consistent, high-quality care. Uphold safeguarding, confidentiality, and all regulatory standards.
