Key Responsibilities: Support hospital discharges by coordinating with healthcare providers and community resources. Conduct assessments and develop care plans that promote independence and well-being. Work collaboratively with individuals, their families, and multidisciplinary teams to ensure smooth transitions from hospital to home. Apply a strengths-based approach to support individuals in achieving their personal goals. Navigate complex cases, including those involving the Court of Protection, ensuring legal and ethical standards are met.
