About The RoleWe are offering a great opportunity to join our team providing personalised advice, guidance and support to people diagnosed with dementia and their carers Location: Community based in West Sussex Northern (Horsham, Crawley and Mid Sussex) with some homeworking elements. Contract: 12 month Fixed term contract.As a Dementia Adviser, you will have the rewarding opportunity to provide support, information, and guidance to people with dementia and their carers; helping to maintain their independence, improving their sense of well-being, and putting them in more control of their lives.The service is unique to each service user as it is based upon their personal circumstances and support needs. You will offer support to your clients in a variety of ways, whether in the client's own home, or at other locations in the community, face to face, by phone, letter, or email. You will also assist service users to access other services, providing signposting and referrals. 

• Supporting people affected by dementia, families and carers to access vital services. 
• Improving people’s sense of well-being, enabling them to have more control over their lives. 
• Assisting with the identification of needs, providing information and access to relevant services. 
• Promoting our amazing service and building relationships with a range of health and social care professionals.
