Therefore, if you are interested, please submit your application as early as possible.About the roleAre you passionate about making a positive difference in the lives of people affected by dementia?We have an exciting opportunity for you to join our team as a dementia adviser covering the Carmarthenshire area.The role is primarily providing person-centred support for people affected by dementia. You will be the first point of contact, providing information about our services and sign posting to other organisations offering support across the area. You will triage referrals for our local Dementia Support Service and work closely with our community-based Dementia Advisers to ensure people affected by dementia are well informed and well supported. The role also provides support for managers across our region and you will be well supported by your line manager in prioritising and planning your workload. You will attend meetings, both face to face and virtually, with both internal and external colleagues across the area. The role also provides support at our local activity groups, as required, as well as supporting our Dementia Voice group which provides people with lived experience of dementia the opportunity to share their opinions and help shape both our local and national services.If you are interested in working as part of a fast-moving, responsive team, absolutely committed to providing excellent services to people affected by dementia, we want to hear from you.About you - You will have a good understanding of the needs of people with dementia and those who care for them and be able to demonstrate a person-centred approach.
- You will need a broad understanding of both national and local statutory and voluntary services and how they can support people affected by dementia.
- You will be comfortable connecting with others and talking to a wide range of people including people affected by dementia, health and social care professionals and related statutory and voluntary organisations.
- You will have good communication and organisational skills, along with a sound knowledge of IT systems to record data, manage referrals and communicate by email.
- You will be a team player, supporting colleagues across our services and be confident to ask for help yourself when needed.
