This is an excellent opportunity for an experienced pharmacist to work within a supportive, multidisciplinary primary care team, contributing to safe and effective medicines management and improving outcomes for patients with complex or long-term conditions. Key Responsibilities Undertake structured medication reviews for patients with chronic or multiple long-term conditions. Provide expert advice on medicines optimisation, safety, and cost-effective prescribing. Support GPs and nursing staff in reviewing repeat prescriptions, reducing polypharmacy, and preventing adverse interactions. Assist in the management and monitoring of high-risk medicines.
