Specialist Physiotherapist Band 6Hours: 22.5 hours Per Week Salary: .60 ( FTE) Depending on ExperienceAn exciting and rare opportunity for an experienced Physiotherapist to join our specialist Allied Healthcare team they will be based in the community to work as a member of multi professional specialist palliative care service to provide a high standard of Physiotherapy assessment intervention and evaluation for patients and carers referred to the service. This includes an in-reach service to patients admitted to the Inpatient Unit assisting in discharge planning and therapeutic interventions.We are seeking an enthusiastic and highly motivated individual to join the multi-disciplinary team in the provision of specialist palliative care to patients and their families.The post holder will need to be able to work autonomously and be responsible for complex holistic assessments and planning care to meet patients practical psychological spiritual and social needs. They will work closely with primary and secondary care professionals and Hospice colleagues and good communication and team working skills are essential.
