OverviewDo you want to work for a caring organisation that believes in people Do you want to make a differenceAs a Recovery Worker with Change Grow Live youll work with people who use our services helping them get to where they want to be.An innovative and exciting specialist service for homeless or those at risk of homelessness is being provided in partnership with Change Grow Live- Spectrum Herts County Council and Hertfordshire Mind NetworkWe are pleased to announcethat we are looking to recruit a staff member to jointhe dedicated multi-disciplinary teamsacross Hertfordshireto support rough sleepers or those at risk of homelessness with harm reductionandengagement into treatment services.Change Grow Live services and staff will provide wraparound support. We are looking for an enthusiastic individual and friendly face to help support people in leading healthier lives.You will focus on engaging with people who have a housing need alongside substance misuse. You will have some experience of engaging with individuals with multiple and/or complex needs and demonstrate a determined approach to finding solutions.Where: HatfieldFull Time Hours: 37.5 per weekFull Time Salary Range: 27861.26- 32002.35 (based on full time hours pro rata for part time)Allowances: Outer Fringe Weighting 694.25 (pro rata)Contract: Fixed Term Contract until 31st March 2026.*Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles the salary and payments will be pro rata based on contracted hours.ResponsibilitiesAbout the role:Establishing effective communication and referral pathways those individuals who are homeless and in contact with other health and care agenciesWork closely with the local authority Health & Social care sector partners and any provision linked to Homelessness and improve joint working attending multi-disciplinary meetings when as and when requiredSupporting service users from point of entry into the service and through their treatment/recovery journeyProviding screening assessment and recovery planning and onward referralCoordinate the transfer of any accommodated out of area individual. This may include referral to and liaison with their new drug and alcohol service and pharmacy and provision of bridging prescription until an appointment and prescription is provided. Promoting carer service user and community involvementProviding advocacy for access to partnership servicesWorking with service users to support social (re)integration enabling them to lead meaningful and purposeful lives: promoting recovery resilience peer support and self-determinationAbout you:Prior knowledge and experience of working with people who are homeless sleeping rough or sofa surfingHave a strong knowledge and understanding of substance misuse issues Experience of engaging with a wide variety of individuals with a variety of backgrounds and needsBe an excellent communicator both verbal and written with good IT skillsProactive and able to work on own initiative with excellent time management and prioritising skillsHave a good working knowledge of mental health interventions services and good practiceBe a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals agencies and internal/external stakeholdersAbility to manage change successfully in a way that prioritises the needs of service usersUnderstand the importance of information governance processes and commit to follow and apply all necessary safeguardsWhat we will give to you:25 days holiday ( bank holidays) rising by 1 day for each years service Capped at 30 daysPaid Wellness hour each week along with a Wellness hub and Employee Assist ProgrammeContributory pension schemeEye test vouchersA great selection of benefits incl. discounts for shopping gym membership cycling equiptment cinema holidays etcA friendly and supportive team Training career development & progression opportunitiesElidgability for participation in our Refer-A-Friend scheme.Please ensure that when completing your application form and supporting statement you reflect on the details outlined in the job description.
