This additional resource will enable early identification of the needs of children and young people aged 5 to 19 (up to 25 for SEND) within and outside of a school setting. You will work with children and young people individually and in groups to promote healthy lifestyles and resilience and improve public health outcomes.Your role and responsibilitiesYou will:support the delivery of the Healthy Child Programme 5-19 yrs by directly supporting the needs of children young people and their familiesthrough multiagency working effectively safeguard children and young people which will include participation in safeguarding meetings and contributing to assessment processeswhere required be the key contact with the family and liaise with other professionals to provide a joined-up responsehave a key role in delivering the Healthy Child Programmesupport health needs of school age children and their families through a Public Health approachpromote health and wellbeing to ensure each child and young person is supported to reach their potential delivering meaningful support to children and young people to achieve positive outcomes.
