Community Matron – Rosedean House Surgery Location: Community-based – Rosedean House Surgery & patient homes Flexible working: Full-time (40 hours) and Part-time can be considered Community Matron – Rosedean House Surgery Rosedean House Surgery is seeking an experienced Community Matron to join our primary care team. The Community Matron will support patients of all ages with diverse and often complex health needs across the Liskeard community. They will provide personalised, holistic care for adults with long-term conditions, frailty, and multiple comorbidities — helping patients live well at home, in care homes, or in other community settings. Given that a significant proportion of our population is older, this role will include a focus on supporting frail and elderly patients, while also addressing the broader needs of adults of working age with complex or chronic conditions. You will be someone who enjoys building strong, trusting relationships with patients and their families. Experience in supporting older adults — including frailty, geriatrics, care homes, or complex long-term conditions — is highly valued, while also recognising the broader needs of adults of working age.
