What youll do Creating support plans with your customers and coordinating our small team of Support Workers working to the plans.Supporting customers with complex mental health, learning disabilities and/or dual diagnosis.Helping customers with daily living activities such as making meals, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation if they are able.Carrying out risk assessments, support planning, goal setting and regular reviews.Cuppas, chats and catch ups.
