The Countywide Community Reablement Team is part of the wider Maximising Independence Service, we are looking to recruit a proactive and dynamic individual to join us, as a Reablement Coordinator in a fast paced and exciting team.

The Reablement Co-ordinator post will be based at Home Brewery Building but with the ability to work in a Hybrid way having the flexibility to work at different locations, whether that is at home or in the office whilst meeting the needs of the service and will cover the whole county.

As part of a very supportive team, and one of four Reablement Co-ordinators within the service, you will ensure the efficient and effective rostering of a workforce of 37 FTE Senior Reablement Workers, You will be expected to manage capacity so that the service is able to accept all new referrals into and out of the service in a timely manner.

You will receive referrals into the service via electronic systems and be able to manage and plan capacity and coordinate resources vital for maximising the efficient running of the service. You will need to have excellent communication, interpersonal skills and IT skills, being able to remain calm under pressure whilst being able to prioritise multiple demands; working effectively with a variety of professionals and departments to ensure the best outcomes for the people we support and timely progress through the re-ablement period. 

In return, you will be supported through regular team meetings, supervision both formally and informally to develop your skills, build expertise and take advantage of the extensive training opportunities to advance your career within the service and the Council. 

This post is 37 hours with participation as required with the on-call rota at weekends and Bank Holidays, for which an enhancement is paid. Hours of work are Monday to Thursday 8.30am-5pm, Friday 8.30-4.30pm
