The South START Reablement Team is part of the wider Maximising Independence Service, we are looking to recruit a proactive and dynamic individual to join us, as a Reablement Coordinator in a fast paced and exciting team covering the boroughs of Broxtowe, Gedling, and Rushcliffe.

The Reablement Co-ordinator post will be based at Trent Bridge House but with the ability to work in a Hybrid way, having the flexibility to work at different locations, whether that is at home or in the office whilst meeting the needs of the service and will cover the whole county.

The post holder will be responsible, as one of five people, for the efficient and effective rostering of a workforce of 70 plus people and a service user group of approximately 80 individuals across all three localities. You will be expected to manage capacity so that the service is able to accept all new referrals into and out of the service in a timely manner.

You will receive referrals into the service via electronic systems and be able to manage and plan capacity and coordinate resources vital for maximising the efficient running of the service. You will need to have excellent communication, interpersonal skills and IT skills, being able to remain calm under pressure whilst being able to prioritise multiple demands; working effectively with a variety of professionals and departments to ensure the best outcomes for the people we support and timely progress through the reablement period.

In return, you will be supported through regular team meetings, supervision both formally and informally to develop your skills, build expertise and take advantage of the extensive training opportunities to advance your career within the service and the Council.

As part of the out of hours support, you will be expected to re-roster at short notice to cover for sickness, bad weather and unforeseen circumstances, to ensure business continuity.
