The Clinical Lead will support the Operational Team Lead in leading a multidisciplinary team in their deployment in the delivery of a clinically effective Nursing Service, whilst providing assessment, treatment and Care Co-ordination to patients in the local community who are referred to the Community Mental Health Team.

Responsible for the pro-active case management of a wide range of patients many of whom have complex needs.

Accountable to the Health Board Nurse Director and the Nursing and Midwifery Council (NMC) to provide safe, high-quality care to patients by implementation of policies/ standards and guidance produced both national and locally. These will include from NMC, Welsh Government, Powys Teaching Health Board, and others.

Main duties of the job

The person appointed will be expected to undertake a range of duties to include:

  • Being responsible and accountable for an identified case load,s, prescribing, providing supervision to the team, liaising with partner agencies, dealing with concerns, chairing and engaging in MDT meetings, Monitor standards of practice within the team, Recruitment and appointment of staff, absence management and the deployment and allocation of duties.
  • Consistently work with the team to ensure standards of patient care are high and maintain safety and quality of the patient experience.
  • With in the team lead the implementation and monitor local and national nursing policy, procedures, and guidelines to support best practice and ensure they are adhered to together with current legislation.
  • Using expert knowledge of managing and delivering care for patients with highly complex and unstable health needs within the community setting and ensure the delivery is safe, efficient, and effective.
  • Responsible for the accurate recording, timely collection, completion, submission of relevant information to support patient care, operational management, nursing staff and skill mix, service development and audit, analysing and interpreting this information to inform leadership within your area of responsibility.
  • Undertake a training needs analysis for nursing within your team, and Implement agreed training plans
  • Actively participate in the research and development agenda

Qualifications and/or Knowledge

Essential criteria

  • Live registration with the NMC as Registered Nurse
  • Degree/Diploma in Nursing
  • Post registration qualification / significant experience in Mental Health
  • Management/ Leadership qualification or equivalent preparation/experience
  • Knowledge and skills to manage patients with complex needs
  • Knowledge of current Nursing issues within NHS care systems
  • High level of knowledge of NMC code of conduct and professional standards/guidance

Desirable criteria

  • Independent Prescribing Qualification
  • MSc Advanced Clinical Practice

Experience

Essential criteria

  • Experience at management level demonstrating ability to deliver improvements to service usercare
  • Experience of proactive caseload management of wide range of patients

Aptitude & Abilities

Essential criteria

  • Ability to use computer systems (email, word processing, spreadsheets)
  • Significant clinical experience in assessing, planning, implementing, and evaluating care for the patient group of the caseload
  • Well-developed verbal and written communication skills
  • Effective multi agency team player, including the ability to engage positively with carers
  • Demonstrable ability and track record of acting with care, compassion, promoting dignity and respect, role modelling the highest standard of professionalism
  • Ability to motivate, enthuse and encourage innovation within the team through effective communication skills
  • Ability to critically review concerns (incidents, complaints etc), to draw reasoned conclusions and write a clear report
  • Proven ability to work in complex situations with many differing partners
  • Ability to implement change effectively
