This post holder will work as part of the cardiac rehabilitation team, dealing with cardiac patients throughout their care pathway from acute hospital admission to providing advice and support after discharge.

This post will be responsible for their own caseload and will supervise the day to day workload of the cardiac rehabilitation team.

There will be some administrative responsibilities as delegated by the team leader.

This post has no direct budgetary responsibilities



Main duties of the job

Under direction of the Lead Specialist Nurse deliver care to defined case load of patients requiring specialist care.

To work as a member of the cardiac rehabilitation team to provide nursing care to patients with a recent diagnosis of heart disease.

With the additional specialist knowledge required to assess, plan. implement and evaluate holistic individualised patient centred care in a

safe and caring environment whilst acting as the patients advocate and maintaining privacy and dignity.

Provide support advice and education to patients and carers to enable

them to understand and manage their cardiac condition and reduce the risk of further cardiac events.

Provide advice and support to junior staff and learners in all aspects of the delivery of high quality care.
