About the Role:

As a Social Worker in the Preparing for Adulthood, Learning Disability and Autism Team, you will:

Assess individual needs and develop personalised support plans.

Coordinate with families, caregivers, and other professionals.

Advocate for clients to access necessary services and resources.

Provide ongoing support and monitor progress towards independence.

Requirements:

Qualified and registered social worker.

Passionate about making a difference in the lives of adults.

Requirements:

Degree in Social Care

Experience in adult's safeguarding

Social Work England Registration
