Adult Support Coordinator

The North Transfer of Care Team covers Hinchingbrooke, Peterborough City and Queen Elizabeth (King's Lynn) Hospitals and small rehabilitation wards and a couple of health interim Care Home placements. We are a friendly and busy team who are passionate about supporting people to receive the right support upon discharge from Hospital.

Please note that the main base for this position is Hinchingbrooke but there is an expectation that the successful candidate would join the rota to cover all the main Hospital sites as needed, as well as working flexibility in the working day to visit other Hospital sites on an as required basis. As much notice as possible will be given if this is required.

We do work weekends, but you are given a day back during the week and a salary enhancement too Our staff receive a salary enhancement between 7% and 15% depending on their selected working pattern.

7% for 1 weekend day a month and 1 Bank Holiday a year

8% for 1 weekend day a month and 2 Bank Holidays a year

10% for 1 weekend day a month and 3 Bank Holidays a year

15% for 2 weekend days a month and 3 Bank Holidays a year

The team support adults being discharged from the acute hospitals and rehabilitation settings. The role involves completing Care Act Assessments, Mental Capacity Assessments, Safeguarding enquiries, care planning, working closely with the multidisciplinary team to identify appropriate pathways for discharge and ensuring unpaid carers are supported by having carers conversations. We do this using a strengths-based approach to our practice.

We do work to tight timescales to ensure people are discharged from Hospital and receiving the care they require in a more suitable environment.

What will you be doing?

You will be responsible for completing holistic social care assessments under The Care Act 2014, and Mental Capacity Assessments to determine the level of care and support needed for Cambridgeshire patients/service users being discharged from the acute hospitals, rehabilitation units and those that have been discharged on a health interim pathway in a care home.

You will be the lead professional supporting people and their families with all aspects relating to their care and care planning to support timely discharge from Hospital.

About you

You will need to demonstrate your understanding of Adult Social Care legislation including the Care Act 2014, Safeguarding Adults and Making Safeguarding Personal principles as well as the Mental Capacity Act 2005.

- You will need to be keen to learn and develop in the role.

- You will be expected to work collaboratively as part of a team and demonstrate commitment to making a difference and building relationships.

- You must have the ability to travel to a variety of locations independently and at short notice.

- You must be able to commit to working in the office 5 days a week during the Probation period so as a team, we can support your learning/development.
