RNLD CHC Nurse Assessor

We are seeking an experienced and dedicated LD CHC Nurse Assessor to join our team on a contract basis.
Our client is looking for someone who can provide high-quality assessments of patients with learning disabilities requiring Continuing Healthcare (CHC) funding, particularly around Joint Funded Care reviews.
Qualifications:
- Must hold current NMC registration as either RNLD or RMN
Skills Required:
The ideal candidate should have excellent assessment skills, including the ability to identify the patient's needs accurately.
They must also possess good communication and interpersonal abilities that will enable them to work effectively with other members of staff.
Applicants must demonstrate their expertise regarding Continual Health Care processes; this includes knowledge about National Frameworks used within England's healthcare system.
Responsibilities:
As an LD CHC nurse assessor your responsibilities include;
- Conducting multidisciplinary assessments ensuring accuracy when evaluating individual medical requirements.
- Developing tailored-care plans based on assessed need which aligns with both clinical best practice guidelines and NHS policies/procedures/regulations.
- Ensuring Continuous development/training/knowledge sharing amongst colleagues/senior leadership teams/providers/families/caregivers etc.
