About the role


This is an exciting opportunity for two Dementia Advisors to join a new pilot service delivering a Primary Care Model of support in partnership with Norfolk and Suffolk Foundation Trust.

Current diagnosis rates in Suffolk are below national average rates and yet we know that getting a diagnosis enables people living with dementia to plan for the future and get access to care, symptomatic treatments, and essential information and advice.


Working in one of two GP Practices in Suffolk, you'll be supporting people pre-diagnosis assessment and their carers at this extremely important time.

You'll provide reassurance, support and guidance at a stage where there is usually very little support available.

As part of this role, you will:

  • Provide preassessment support and dispel myths around diagnosis.
  • Provide information about the memory assessment process.
  • Highlight the importance of diagnosis and that is not natural part of aging.
  • Work in partnership with the Clinical Dementia Leads.
  • Attend weekly Multi-Disciplinary Meetings (MDT) with primary care colleagues.
  • Provide emotional and practical support.
  • Work alongside the current Dementia Support Service in Suffolk.
About you

We are hoping to welcome a team of two approachable and knowledgeable Dementia Advisors to work in Suffolk.

We are looking for

  • Working experience of assessing people face to face with an ability to assess their needs.
  • Deliver advice and guidance with a nonjudgmental approach and outstanding communication skills.
  • Strong organisational and time management skills to manage your caseload of clients effectively.
  • Good IT skills and be able to travel independently across East and West Suffolk.
  • An understanding of dementia and the needs of those living with dementia and their carers would be an advantage but if you have had experience supporting with other challenges this would be taken into consideration.
