The Clinical Directorate is delighted to be recruiting a Lead Pharmacist. The position will provide support to the Head of Medicines Management to deliver the safe and secure management of medicines in the Trust, including the implementation and reporting of their use providing evidence for inspections by regulators and auditors.

The Lead Pharmacist will take a lead role in the provision of safe systems of work aligned with the necessary regulation and mandatory compliance frameworks.

Responsible for the assurance, evaluation, and improvement of patient care through their responsibility and accountability of Medicines Management activity in the Trust. They will be also be required to support and assist in the development of a Medicines Optimisation Strategy.

Provide highly specialist advice about the use of medicines to clinical staff to numerous professions such as doctors, paramedics, nurses, mental health nurses, ambulance clinician's, pharmacists and pharmacy technicians. They will respond to complex queries regarding combinations of medicines and their applications in specific patient presentations, to provide the highest level of patient care within the Trust.

Additionally, they will lead on the development and review of Patient Group Directions (PGDs), ensuring the appropriate PGDs are in place, are in date, and up to date. This will also include the development, implementation, and monitoring of a PGD policy pharmacist.

Main duties of the job

  • Providing a high quality and comprehensive medicines optimisation service as part of the Clinical Team
  • Working collaboratively with the Head of Medicines Management
  • Supporting the development of best practice and safe medicines management and optimisation systems
  • Supporting the delivery of medicines management audits and reporting
  • Acting as lead coordinator for Patient Group Directions (PGDs) and Written Instructions (WIs)
  • Collaborating with and supporting the clinical team in the development and growth of independent prescribing
  • Leading identified medicines management projects
  • Supporting the development of the WAST Medicines Formulary to support effective use of medicines
  • Supporting training of various staff groups in medicines management, including student paramedics studying in Welsh Universities.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
