his is a short term post, approx. until end of September, as Social Worker in the 16-25 Disabilities Team, which is part of 0-25 Service in Children's Services. Social Workers in this team work under adult legislation to consider eligibility under The Care Act, and service provision as appropriate. We work alongside Education colleagues and our Preparing for Adulthood Team with much joint working.

What will you be doing?


• You will have a caseload of young people who are transitioning from Children's Services or who have already recently done so. With a holistic approach, you will be planning for the next stage in their life which may mean changes around education, provision, budget, maybe moving away from home for the first time, considering outcomes and developing skills
• You will carry out Assessments under The Care Act, seeking funding for care packages and reviewing services with a multi agency approach as appropriate


• You will help families understand that their young people are now adults and are able to make their own choices for greater independence. Navigating the transition process into adulthood with different professionals and legislation involved. We help families to understand this process, while helping them understand that their young person is at the centre of our work. This can be challenging because a family member has often been used to being the decision maker.


• You will participate in and manage our duty system by being the Team point of contact for Social Workers and Social Work Assistants giving direction about referrals if needed, understanding when to seek Management oversight for complex decision making What we need from you


• You will be a qualified social worker with relevant Social Work England registration to practice and a good understanding of asset and strengths-based approaches to social work.


• You will demonstrate specialist up to date knowledge of the service area of Adult care and disability and be able to demonstrate knowledge and experience of using relevant legislation, regulations and codes of practice inc The Care Act, Mental Capacity Act, EHCP Code of Practice, Safeguarding Adults Policies and Procedures, Deprivation of Liberty responsibilities


• You will understand the 3 Conversations way of working which is our model in South Gloucestershire adult care


• You will be able to demonstrate skills, knowledge and experience to be able to assess, plan, provide and review for the most vulnerable young adults in the community. This includes the ability to assess and manage risk.


• To have a good understanding of the possible funding changes from the 18th birthday i.e. from Childrens to adults, CHC etc
