Adferiad delivers a flexible and coordinated response to the exceptional circumstances faced by people with co-occurring mental health and substance use conditions and related issues. Vulnerable people facing complex life challenges need consistent and seamless support to ensure they remain engaged with vital health and wellbeing services – and to prevent them becoming disenfranchised and isolated. Adferiad harnesses the talents and experience of long-established and leading Welsh charities CAIS, Hafal and WCADA. Our combined expertise in the fields of substance use, mental health, housing, criminal justice, and employment support enables Adferiad to meet the needs of our most vulnerable with a single, unified, and comprehensive approach.

Role Purpose:

Adferiad provides a 24 hour temporary Supported Accommodation to individuals with a serious mental illness in Llanelli, Carmarthenshire. The Llanelli service is delivered within a shared accommodation for six adults or in people's own homes in the locality. Our clients have experience of serious mental illness and need a medium level of support to help with their recovery. The service is staffed 7 days a week, and is offered to clients on a short to medium term basis according to need.

The aim of the Service Manager and staff team is to encourage and enable clients to reach a greater level of independence and then to facilitate their move on to more independent living in the community, supporting the wellbeing initiatives as identified within Welsh Government policies and the Social Services and Well-being Act (Wales The role of the Service Manager is to ensure the timely, efficient and effective management and delivery of services within the project in line with our commitment to the principles of empowerment, self-management and recovery and ensuring compliance with internal and external quality standards inspection and regulation (including CIW).

Main Duties / Responsibilities:

  • To work with Commissioners, Community Mental Health Teams and other partners to manage & deliver a high specification service and regulations.
  • The Service Manager will take the lead on processing of approved referrals, following procedures for completing new assessments, risk assessments and creating individual person-centred support plans and keeping Carmarthenshire County Council Commissioners & relevant team informed of decisions and progress.
  • Follow procedures for conducting 3 monthly support plans reviews – or as and when required due to changes in individuals' needs or circumstances, ensuring risk assessments, support delivery plans are kept up to date and are relevant.
  • Work with individuals to achieve their outcomes and the outcomes of the project, using a person centred approach to assessment and support planning employing creative and innovative methods to meet these outcomes and, through the Manager's & staff's experience and resources, bring added value.
  • Ensuring support is provided based on an individual's needs, desires, and personal preferences to promote wellbeing and independence whilst adhering to the services Contract and all relevant policies and procedures – internal and external.
  • Making sure individuals receiving support are enabled and empowered to exercise informed choices about their lives where possible, to enhance their social skills and encourage participation in the community.
  • To ensure individuals are encouraged to provide their views on the service, evaluation surveys and to be involved in local & Adferiad Service User consultation groups.
  • Responsibility for the management and co-ordination of rotas, ensuring flexibility within the team's capacity and rotas to meet the changing needs of individuals and project
  • Work flexibly, actively participating in the rota including sleep-ins.
  • Ensuring client files and records are of a high standard, up to date and always prepared for funders' audits, validation checks, Adferiad Quality Standards and CIW Inspections.
  • Leading on performance of the staff team; allocate tasks, agree objectives, set out plans and work methods following best practice and ensuring the service is accountable.
