Job summary

The post holder will work as part of the countywide Early Supportive Discharge/Neuro Service.

The Early Supported Discharge Service enables the accelerated discharge of Stroke and Acquired Brain Injury patients usually to their own home or normal place of residence from district or community hospitals.

Job responsibilities

The post-holder will provide administrative support for the key tasks of the ESD Service, including waiting list management (via Excel and RiO); monitoring of service quality improvement plans and service performance targets.Provide comprehensive clinical administrative services, including telephone duties, typing of correspondence and reports and the organisation of client appointments including creation and posting of relevant letters and forms.Taking telephone calls from ESD clients/carers who may be distressed or have high expressed emotion and manage these appropriately and signpost them to appropriate clinicians and services.To minute ESD business meetings and other meetings as required and carry out any actions as required.To provide a point of contact for professionals from within the Trust and across a wide range of external agencies, families, carers, and young people, ensuring effective communication, liaison and courtesy at all times.Maintenance of administrative files and records of referrals, attendances and discharges.Maintenance of client files - ensuring confidentiality.To type reports, letters, minutes and forms etc. maintaining accuracy, appropriate layout in line with Trust policy and presentation.To assist in data input for research purposes and obtaining information from the RiO system for audit and Service Quality Improvement Plan purposes.To assist with the preparation of resources for training in terms of printing, preparing PowerPoint presentations and any other general support.To maintain Excel spreadsheets pertaining to clinical activity within the ESD service as required.To prepare text, tables and diagrams for publication.To assist in ensuring relevant files, papers, refreshments etc. are available for meetings.To take initiative where necessary, prioritising the workload to ensure deadlines are met.To maintain confidentiality and to ensure working practices are secure.To assist with the provision of administrative cover in the event of sickness absence and annual leave, travelling to other units within the Trust if required.Adhere to all Trust policy procedure and guidelines.Participate in regular supervision and annual appraisal processes, in line with Trust policy.Attend and maintain appropriate levels of training within the Trusts mandatory training framework.Attend training courses as agreed with the Line Manager.This post is recognised as a DSE user and comes under the DSE policy Attending meetings as appropriate.
