Your main responsibilities as a Social Worker:
- Carry out comprehensive assessments to determine individuals' needs, ensuring all assessments are in line with the principles of the Social Services and Wellbeing Act.
- Collaborate with multidisciplinary teams to develop and implement personalized care plans that meet the unique needs of each individual.
- Assist individuals in accessing a range of community resources to support their wellbeing and independence.

- Social Care Wales registration
- Eligible to work in the UK
- Hold a full UK licence
