What is the job function?

An exciting opportunity has arisen for sessional nurse to support the Health and Wellbeing Team. You would have the opportunity to work with young people in secure and residential care. You would be assigned work by the Senior nurse to support the health assessments and the delivery of high standards of care for young people with a range of health conditions, risk taking behaviours, neurodevelopmental disorders and mental health needs. This is achieved by using your clinical judgement and public health knowledge to identify health needs and detail a health action plan. You will provide guidance to staff and support the implementation and review of the health action plan.

Responsibilities would be delegated by the Senior Nurse

  • To undertake comprehensive health assessments for young people assessing their physical and emotional health, medication and lifestyle factors. Providing a detailed report and action plan to inform the young person's care planning.
  • To assess young people with health concerns and provide guidance to staff on how to support the young person.
  • To facilitate the outreach NHS services within Kibble e.g., sexual health, immunisations and support the young people to access them.
  • To support neurodevelopmental assessments and provide a written report liaising with CAMHS/ FCAMHS clinicians.
  • Demonstrate knowledge of the determinants of ill health and appropriate interventions. Be aware and give advice on issues relating to health education/promotion and health improvement.
  • Support the implementation of the Electronic Medication System across the services

Communication and Relationships

  • Responsible for the development of therapeutic relationships with our young people.
  • Communicate highly complex information regarding the management of mental and physical health problems to colleagues and other disciplines/professions, young people and carers in a manner that is clear and easily understood.
  • Communicate with young people and their relatives using a variety of communication skills.
  • Communicating with young people with impaired communication skills due to their developmental disorder or mental health.
  • Be able to communicate with all colleagues in a professional manner.
  • Be able to communicate with colleagues in other organisations in a professional manner.
  • Practice and always promote confidentiality upholding NMC code of conduct.
  • Support distressed, upset, angry or anxious young people.
  • Use communication skills to de-escalate conflict situations ensuring the safety of self and others.
  • Ensure that written communications meet the NMC Code to keep clear and accurate records.

Qualifications, Skills & Experience: Essential

  • NMC Registered Nurse
  • Minimum of 2 years' Band 6 experience in a relevant clinical setting e.g., school nursing, working in the fields of primary care, paediatric A&E.

Qualifications, Skills & Experience: Desirable

  • Desirable in addition to essential, Registered Childrens Nurse, Specialist Community Public Health Nurse (SCPHN), Learning Disability Nurse (LD)
