This is a fixed term post expected to last until

Varied shift pattern over 7 days a week 7.30am - 1pm and 5pm - 9.30pm

Grade 4 - 23,114 for 35 hours per week for 52.14 weeks per annum equating to 21,864.59 pro rata.

As a reablement support worker, you will be directly working with therapists and other professionals to implement outcome/ goal focussed care plans to maximise service users independence. You will receive ongoing support and training to develop skills and competencies central to your role.

We are looking for enthusiastic, motivated individuals, who are willing to learn new skills and develop their knowledge. Understanding of the ethos of reablement- short term intervention to promote independence is essential, as are good communication skills both verbal and written.

This is an excellent opportunity to play a vital role in the development of a new service that will make a positive difference to the lives of people in Pembrokeshire. It is a role that provides a great deal of job satisfaction in seeing people improve and recover, and being part of a multi-disciplinary team dedicated to continuous improvement and development.
