     The service offers support and intervention to people aged 18 plus, who live with physical disabilities, chronic and long-term conditions, autism, and sensory impairment.     As a crucial member of the team, you'll collaborate with individuals, families, and caregivers, conduct Care Act 2014 assessments, evaluate needs, and manage both short-term and long-term cases.
