Our Associate Clinical Case Managers support some of the most vulnerable children, young people and adults following life changing injuries such as acquired brain injuries, spinal cord injuries, complex orthopaedic injuries and more.Our Associate Case Managers operate UK wide, and due to growth; we are seeking clinically qualified professionals to join our network on a self-employed basis:What youll be doing as Associate Clinical Case Manager:managing a case load of clients through their rehabilitation.leading on assessment in the home and their rehabilitation and goal setting.signposting and advocating for what they need, and assessing what needs to be in place for the people you support to live a fulfilled life, whether that be accessing services, reintroduction into their communities and social lives, accessing work or meaningful activity and so on.Managing all administration in relation to cases, including sending instructions to third parties and writing and reviewing reports.To be successful in this role, youll have proven clinical experience and a valid clinical registration. Youll have previous experience putting together and implementingrehabilitation packages and strong relationship building skills. As this role is field based, you will be a driver with access to your own vehicle and able to travel across the UK. We believe this is because of their extensive knowledge of catastrophic injury, rehabilitation and wider support services but also because they care about rebuilding lives and creating a fulfilling future for those we support.Working with Bush & Co you can expect to work with like minded people who challenge the status quo, care about what they do, learn from each other and never take their eyes off the end goal; we think thats pretty special.As an associate case manager you will work with people who have suffered catastrophic injury to:Assess their needs, understand them pre-injury and identify their needs post-injuryPut plans in place based on these needs for a successful rehabilitation with clear, manageable goalsImplement the plans alongside the client (including the parent or carer of children and young people)Coordinate the relevant care and support whether that be accessing statutory services, therapy and rehabilitation, financial support as well as educational needs, vocational support, hobbies and lifestyle choicesMonitor progress and respond to changing needs.You may also have experience in the following: Occupational Therapist, Mental Health Occupational Therapist, Home Modification Specialist, Assistive Technology Specialist, Sensory Integration Therapist, Work Capacity Evaluator, Certified Occupational Therapy Assistant (COTA), Industrial Rehabilitation Specialist, Rehabilitation Services Director, Community Integration Specialist.REF-214078
